Check your eligibility

To check your eligibility for hearing services we require your first name, surname, date of birth and eligibility number (i.e. Centrelink, DVA). The system will then check if you are eligible. This only takes a few minutes.

If you are eligible and you would like to proceed, please provide your contact information address, e-mail and phone number. 

This finalises the online application step. A welcome pack and a service provider listing will be sent to you.
