Privacy and your personal information

The Department of Health is committed to protecting your privacy in accordance with the
Australian Privacy Principle guidelines.

Australian Privacy Principle 5 Notification

The Department of Health (the Department) is collecting this personal information about you to determine eligibility for and administer the Hearing Services Program.  The Department can be contacted by telephone on 
(02) 6289 1555 or freecall 1800 020 103 or by using the online enquiries form at www.health.gov.au.

If you do not provide this information, the Department will not be able to provide you with hearing services under the program.

The Department may disclose your personal information to, and receive personal information about you from, Services Australia, Veterans’ Affairs, Social Services and the Defence, Employment.  The Department may also disclose your personal information to, and receive personal information about you from, your hearing services provider and medical practitioner for the purpose of administration and clinical delivery.

The Department has an Australian Privacy Principles (APP) policy which you can download from the Department's website.  You can obtain a copy of the APP policy by contacting the Department.  The APP policy contains information about

  • how you may access the personal information the Department holds about you and how you can seek correction of it and
  • how you may complain about a breach of the Australian Privacy Principles

The Department is unlikely to disclose your personal information to overseas recipients.
