Disability Employment Services

Information for Disability Employment Services (DES) participants and providers.

DES – Employment Support Service and Disability Management Service

Both DES Employment Support Service (ESS) and Disability Management Service (DMS) participants are eligible for the Hearing Services Program (under Part III) of the Disability Services Act 1986.

To apply for the program, DES providers are required to download and complete the application form available from the Hearing Services website. Please ensure that the participant’s personal details and DES provider information is all complete.

When completed, DES Providers must email the application forms to hearing@health.gov.au on behalf of the client.
